FMPSD Change of Address and/or Phone # Form

Please complete this form if your child is an existing bus rider and you have moved, your sitter has moved or you have changed sitters and require transportation from the new address.

Completing this form does not guarantee transportation will be provided from your new address.  The new address must still fall within the school catchment area and meet the eligibility criteria.  

If you have moved and are looking for your child to ride the bus for the first time, please complete the FMPSD School Bus Transportation Request Form instead.

If you have moved and no longer require transportation, please complete the School Bus Cancellation Form instead.
